Prediction tool for new distant brain metastases within 1 year after stereotactic radiosurgery for a maximum of 3 brain metastases

This tool predicts the occurrence of new distant brain recurrences (DBR) within 1 year after stereotactic radiosurgery (SRS) for a maximum of 3 brain metastases (BM).

To which patients does this tool apply?

Patients with 1 up to 3 BM treated with SRS. The number of BM was determined based on an contrast enhanced T1 weighed MRI. The vast majority of the patients were in a good physical condition.

How was this tool developed?

The nomogram is based on data of 361 patients, treated at VUmc Amsterdam with SRS for a maximum of 3 BM from several solid primary tumors.[1] The model was built using multivariate logistic regression and Receiver Operating Characteristics (ROC) analysis. The predicted outcome is DBR within 1 year after SRS for BM. There was no missing data, because data was manually checked per patient in medical charts. A multivariate analysis was done to identify clinical factors which significantly predicts 1 year DBR: number of brain metastases 1 versus 2 or 3 (p=0.001) and GTV of largest brain metastasis (p=0.045). A nomogram was developed with logistic regression for the prediction of 1 year DBR based on number of BM, age, GTV largest BM, and WHO performance status. Thereafter external validation of the nomogram was performed in 297 patients treated with SRS for a maximum of 4 BM in MAASTRO Clinic, Maastricht.[2]
