Description of the prediction tool for treatment induced dyspnea

This tool predicts the probability that a lung cancer patient will develop treatment induced dyspnea (>= grade 2, CTCv3.0) at any time point during or at maximum 6 months after radiotherapy treatment.

To which patients does this tool apply?

The tool is developed for NSCLC patients, stage I-IIIB. Patients have to be treated with high-dose conformal radiotherapy alone (<=3Gy per fraction) or high-dose conformal radiotherapy combined with chemotherapy (sequential or concurrent). They do not have distant metastasis at the time of diagnosis.

How was the initial tool developed?

The prediction model is based on data of 259 lung cancer patients, treated at MAASTRO clinic with high-dose conformal radiotherapy alone or combined with chemotherapy (sequential or concurrent) and built using logistic regression. The predicted outcome is the probability that a patient will develop dyspnea ≥ grade 2, CTCv3 at any timepoint during or at maximum 6 months after radiotherapy treatment.

Missing values have been substituted with the mean, median or the modus, whichever was appropriate.

The initial inputs consisted of mean lung dose, existing cardiac comorbidity at the start of radiotherapy, smoking status, type of chemotherapy, age, gender, forced expiratory volume in 1 second adjusted for gender and age (FEV1, in%), lung surgery performed in the past before radiotherapy, world health organization performance status (WHOps), tumor location, lung volume, prescribed tumor dose expressed as equivalent radiation dose in 2 Gy fractions corrected for overall treatment time (EQD2,t) using a/b = 10 Gy and accelerated repopulation kick-off time of 28 days, overall treatment time, and the level of dyspnea at the start of radiotherapy. After a wrapper-based feature selection, the following variables remained in the final model: baseline dyspnea score at the start of R(CH)T (radio(chemo)therapy), cardiac comorbidity, tumor location, Forced Expiratory Volume in 1 second, and chemotherapy.

The performance of the model is expressed as the AUC of the ROC. The maximum value of the AUC is 1.0, indicating a perfect prediction model. A value of 0.5 indicates that patients are correctly classified in 50% of the cases, i.e. as good as chance. The performance of the model on the training set, assessed with 10-fold cross validation was 0.723. External validation of the model on the two combined external datasets yielded AUC of 0.674.

What information will you need?

  • Dyspnea score before start of radiotherapy treatment (using dyspnea scale CTC v3)
  • Cardiac comorbidity, defined as a recorded historical treatment of any cardiac disorder at a cardiology department before the start of RT (radiotherapy), irrespective of its severity.
  • FEV1 (forced expiratory volume in 1 s) before start of radiotherapy treatment, in %
  • Tumor location (upper versus middle/lower lobes)
  • Sequential chemotherapy ( yes/no)
