Description of the prediction tool for survival of non-small cell lung cancer patients

This tool predicts the probability that a patient with non-small cell lung cancer (NSCLC) will be alive at 2 year post-radiotherapy treatment.

Description of the prediction tool for survival of non-small cell lung cancer patients

The tool is developed for NSCLC patients, stage I-IIIB. FDG-PET has to be used as a staging tool. Patients have to be treated with high-dose conformal radiotherapy alone or high-dose conformal radiotherapy combined with chemotherapy (sequential). They have not undergone surgical treatment nor prior chest radiotherapy, nor have distant metastasis at the time of diagnosis.

How was this tool developed?

The prediction model is based on data of 322 NSCLC patients, treated at MAASTRO clinic with high-dose conformal radiotherapy alone or combined with chemotherapy (sequential) and built using Support Vector Machines. The predicted outcome is 2 year survival post-radiotherapy.

The mean value of a variable is imputed if a value is missing. The percentage of missing values is 15% for forced expiratory volume 1 s (FEV1), 2% for World Health Organization Performance Scale (WHO-PS), 11% for gross tumor volume (GTV) and 5% for number of positive lymph node stations (PLNS).

Combinatorial variable selection is performed with a fixed number of variables. A set of five variables with the greatest area under the curve (AUC) of the receiver operator curve (ROC), assessed by leave-one-out (LOO) cross validation, is included in the multivariable model.

The performance of the model is expressed as the AUC of the ROC. The maximum value of the AUC is 1.0, indicating a perfect prediction model. A value of 0.5 indicates that patients are correctly classified in 50% of the cases, i.e. as good as chance. The performance of the model on the training set, assessed with LOO cross validation is 0.74. Two datasets from Ghent (n=44) and Leuven (n=24) are available for external independent validation of the model. Application of the model to these datasets yields an AUC of 0.75 and 0.76 respectively. Validation of the model on a combined dataset (n=68) results in an AUC of 0.76.
