Tool for prediction of survival and local control in laryngeal carcinoma patients treated with radiotherapy alone

This tool predicts the probability that a laryngeal carcinoma patient will be alive or will have local control after delivery of radiotherapy.

Required characteristics to use the tool

This tool was developed for laryngeal carcinoma patients, stage I-IV. Patients have to be treated with high dose radiotherapy alone. They have not undergone surgical treatment or chemotherapy, nor have distant metastasis at the time of diagnosis.

Development

The prediction model is based on data of 994 patients with squamous cell laryngeal carcinoma, treated at Maastro Clinic with radiotherapy alone. This tool was validated on a pooled dataset of 895 patients treated with equivalent treatment in Leuven (Belgium), Amsterdam (VUMC and NKI/AVL Hospital, The Netherlands) and Manchester (UK).

The available clinical factors were age at start of treatment, gender, tumor location (glottic or non-glottic), pretreatment hemoglobin level, equivalent dose in fractions of 2 Gy corrected for overall treatment time (EQD2T), T-stage, and N-stage (N0 or N+). Treatment outcomes were defined as overall survival and local control, both calculated from the start of radiotherapy. Failure of local control was defined as persistent or recurrent local disease after the start of radiotherapy (i.e. the first relapse after therapy).

A multivariate model was developed using the Cox proportional hazards model. Cox regression was used to convert the model output to a probability of treatment outcome. Missing data was imputed using predictive mean matching. The model performance was expressed as the area under the ROC curve (AUC). The maximum value of the AUC is 1.0; indicating a perfect prediction model. A value of 0.5 indicates that 50% of the patients are correctly classified (i.e. as good as chance). Bootstrapping techniques were used to obtain confidence intervals for the model predictions and to correct for over-optimism.

The AUC of the developed models was 0.73 for survival prediction and 0.67 for local control. The application of these models to the pooled validation set yielded an AUC of 0.71 for survival and 0.65 for local control. The application of the models to each external dataset of Leuven, VUMC, NKI/AVL and Manchester, yielded AUCs of 0.68, 0.74, 0.71 and 0.76, respectively for survival, while the validation for the local control model resulted in AUCs of 0.70, 0.71, 0.62 and 0.72, respectively.
